What formation is this play in? I'm making a custom playbook and I can't find it anywhere

There's one in I Form: Normal, I Form: Tight, and I Form: Twins. Didn't search any other formation types besides I Form. Check out SimSports. It's a great resource when making a custom playbook. You can look at all the plays in every formation. it's a little easier than scrolling through the formations on the game.
