Players subbed in at wrong positions

I just paused my game because for some reason I just got an INT and it was my 2nd string QB!?!? I have my depth charts set and I do not have him anywhere except as my second string QB. I can also recall this situation happening the other day also. I looked up and I had a LD in as a defensive end. My depth chart does not have it set up that way and I do not make formation substitutions.

Has anyone else had this issue?! If so, is there a solution to it?

Was the interception after a fumble that was overturned?

Had an occasion where my offence became my defence, and the CPU's defence became their offence until they went back to the huddle.

No. I punted the ball. They came out and went no huddle for 4 plays then bam, INT by my QB. This entire thing is weird!

most likely he is an ATH you recruited who had covering skills the Mass Subs in this game are broken but my Army QB Gunner Skywalker Threw A TD and also Returned an INT for a TD

Yeah I had my starter QB playing on the kickoff and then got hurt on the field. I was pretty pissed off about that. Out for 6 weeks and pretty much my NC down the drained....
