Get Dimer and Floor General ASAP. Ryan Anderson is money. You said Pick & Fade doesn't always work, but with the +2/3/4 Shot Med/3Pt that everyone gets with Floor General, and then the +10/20/30% added bonus with Dimer, you're practically guaranteed the assist when passing to an open Anderson/Gordon/Rivers.

Get dimer, milk the clock until there's about 2 seconds left, pass to a shooter, shot clock cheese. (you don't even need dimer.)

I dribble down close to the paint and send players towards the basket (Triangle + Right directional pad).

They usually get open and I just hit them on the way to the basket. Takes some good timing or you'll get some turnovers. I only added Floor General so far. Probably gonna add Dimer when I get the starting spot from Ty Lawson.

I also experienced this problem initially. Extend the minutes per quarter and feed the scorer/shooter on your team. Guaranteed to raise your assists; you'll be setting the team record in no time.

Use plays for Gordon, Rivers, or Anderson.
It's definitely worth it!
The pick and roll Anthony Davis is the easiest way possible for you to get assists in my opinion but as you said it never works sometimes so I'm gonna suggest
[ Post up your man, back him down and you'll see people cutting to the paint and there you've got to past it's easy assist.. If they don't cut try the cut to the basket play]

The Best way to get assist is to learn the game. Once you figure out what works regarding the passing mechanics things become easy. My Center who doesn't have any passing signature skills and a max pass rating of 50 averages 10 assist per game.
