Well my OD is run from a website. We generally get people an opportunity to present their case. This year, a number of people including myself had to restart games where the playbook glitch came up and they only had a blank play call screen. In that situation, we just asked for a picture to show the blank playbook and it was no big deal.
We have had a few games not save after completed and we generally take people's word for it there. We have a pretty trustworthy group so I don't worry about it much, but some new guys have come in and had some interesting restarts and we've usually just said, you need to provide proof or we can't let you play this game, you take your chances with the sim.
If it becomes a bigger issue, we just boot them and don't wait around. We have suspended players for games for things like that, or unrealistically running up the score/probably cheating on the CPU (we had a guy put up 87 and 700 yards against Boise State with Georgia, including 4 TD in the last 1:30 of the game, no idea how) and so we suspended a player for a game since we are very strict on realistic play, even vs the CPU.
I like the idea of some sort of penalty option in the game, so long as it wasn't something that could get abused, like the commish being able to edit his own players in an OD this year.
