Here's an Option 3 that takes much less time than the previous two options listed below:

Found a fix that only took me about 20 minutes!

I downloaded the roster file LETSGOMETS89 V1 (make sure you get the one with the capital "V" in "V1"; that fake roster turdmonkey is back at it again). And yes, it's based off of OSFullMinors2010V.1

This file only has 25 players on the 40-man roster for each team. This alleviates that lineups issue since it's caused by the CPU trimming down MLB squads from 40 to 25 after Spring Training.

Note: You have to use Franchise mode and not Season Mode. In Season Mode you only have 25 players on your team and everyone else is a free agent instead of being in your minor league system.

So after starting a new Franchise with this roster file, I went ahead a simmed through Spring Training (with injuries turned off for the sim) and advanced to the Regular Season. Lineups were perfect.

Before playing any games, I went through every team's AAA squad and added 15 players to the 40-man roster (top 8 pitchers and top 7 position players, including at least 1 catcher). It took 20 minutes tops. If you want to be more meticulous than I was, it could take a little longer, but it's MUCH faster than other fixes previously posted.

So by filling every team's 40 man roster, if the CPU has an injury on their MLB squad, they can call someone up from AAA, and everyone's also set for September Callups.

Here's some additional notes on settings/methodology and such:

You HAVE TO control all 30 teams. Assign your profile to the team you are playing with, and NO PROFILE to the rest of the teams.

Make sure GLOBAL SETTINGS are set to:
Lineups/Rotations - Manual
Injury Management - MLB Only
Everything else can be set to Auto if you'd like

Start playing the games.

As each day progresses, you will be notified of various injuries around the league. For each injury alert, choose the appropriate action to take based on how long the player will be out (place on bench, 15-day DL, 60-day DL). If the player goes on the DL, the CPU will handle the AAA call-up appropriately.

If an injured player happens to be a starter (and therefore leaving an empty slot in the lineup) you will be prompted that such-and-such a team "Does not have 4 valid lineups". At that point, fix their lineup as you see fit.

If a starting pitcher is injured and you get the "Does not have a valid starting rotation" alert, I just hit auto-fix and the CPU handles it okay.

When you get an alert that a player is no longer injured, and/or off the DL, place him on the bench. Then go to the team's lineups and insert him back in.

And there you have it. Sounds like a lot, but it's really just a couple minutes of maintenance between games. And it does keep you well informed on what's going on around the league.

This was helpful in another thread, so I thought it may be worth its own thread. And perhaps a sticky?

Here's the underlying problem behind the wacky lineups once the Regular Season starts:
  1. During Spring Training, there are 40 guys at the MLB Level for each team
  2. Once you advance to the Regular Season, the CPU automatically trims down the MLB rosters to 25
  3. For some reason, this process of the CPU trimming the MLB rosters down from 40 to 25 is causing all the lineups to be reordered, even if Manual Lineup Control is ON. Doesn't matter if you are controlling 1 team or 30, any Manually Controlled Lineup will still be reordered.
There are two fixes to choose from, both of which require 30-team control and are quite tedious:

  1. Option 1: Before advancing to the Regular Season, go through each team and set their MLB 25-man roster manually. Only 25 players can be at the MLB Level; everyone else needs to be sent to either AAA, AA, or A. You can still have 40 guys on the "40-man roster" (the "40-man" column that has "yes" or "no" next to each player), but you can only have 25 at the MLB Level. OR
  2. Option 2: Advance to the Regular Season and allow the CPU to set the MLB 25-man rosters automatically. Before playing any games, go in and adjust each lineup manually.
So for these fixes, you have to pick your poison:

For Option 1, you have to trim down every MLB roster to 25, but the lineups will remain in the correct order when you advance to the Regular Season.

For Option 2, you don't need to worry about the 25-man roster, but you have to fix all 4 lineups for every team (which also requires fixing some players playing in the wrong position, btw).

I personally prefer option 1.

Furthermore, if someone has the time to implement one of these fixes, create and post a Season or Franchise file for others to download and start from, I'm sure the community would hold you high in their graces

At what point during option 1 due you adjust the rosters from 40 to 25?? Before ST of after simming ST?

I've simmed to the last day of ST before trimming the rosters and it seems to work just fine. Just don't advance to the Regular Season until you finished trimming rosters down to 25.

I'd also recommend turning injuries off through ST. I found that lineups will get screwed up (even with the 25 man roster fix) if someone in the originally set lineup isn't on the 25-man roster.

For those who are currently injured how do we send them to the DL??? I can't find an option anywhere??? Thanks

At the risk of sounding dumb - I don't recall the simmed/skipped Spring training ever resulting in injuries in previous years. That being said, if the DL function to assign injuries can be accessed DURING Spring Training, perhaps setting players who would have been on opening day rosters IRL and in the game (based on ratings) to injured in line with their actual injuries will be one step along the way to paring the 40 players down to 25. I would personally play the odd Spring Training game to get a feel for rookies, or if I am focusing on a team or two for the season, play the Spring game(s) vs rivals. This would allow you to realistically 'cut' your roster down to size before hitting the regular season.

I posted in the main Full Minors thread (the current one) that MLB main site has an injury update under the Fantasy heading. I did save a doc version the evening of April 4 and would be happy to post if anyone would like to use as reference. It is fairly long, and might require multiple posts (maybe one per division).

I will mention again here that the mlbdepthcharts site has an excellent excel sheet from April 5 with the opening day rosters, DL players, and 'next in line' players ticketed for AA and AA.

Aye, i'm going thru each team and updating the 40 man roster and 25 man rosters. I've done the NL EAST,CENTRAL and about to start on the WEST. Basically, I'm taking down notes and putting it into the game. I am doing lineups for LHP AND RHP pitchers. Anybody missing I will putting on my list to the roster makers and so forth. My understanding is that at the end of spring training, you downsize the rosters then go to the season and the lineups stay intact well as far as the 25 man rosters. Help would be appreciated, I will do the WHOLE ROSTER and somebody can show me how to upload it on here so nobody has to go thru what i'm doing. I'll take 1 for the team.
