I believe it was whenever the CPU called Cover-2 defense. The safeties would crash in and allow the wide-receivers to break wide open with ease. I believe in the "hands-on" that Chris did, he said something about it not being there anymore, but I could be wrong.

Does anybody know how the process works? I know Microsoft has to test the demo, but does EA send the demo in advance or do they send it the day it's supposed to release?

Yep. It's just a game for one thing. If I wasn't at work I'd be doing something beside getting upset over a game..let alone a demo.
