I'm sorry for being an idiot or something. But what is the "simple fix"?

There is a fix but if you fix it, when you go play a game, it wont work correctly. If you play a game with your black line setup, the lines will disappear during the game and during a franchise.

In other words dont worry about the black lines

It was the graphic settings.

I had to import a custom graphic setting to play Head Coach 09. That is what caused my issue with M12.

Reset your graphic configuration to default and it should clear up this issue, if you're having it.

It is best to reset the Madden 12 settings to default before applying the above custom settings.
