It's usually how many holes one player is up followed by how many holes are remaining.
For instance a player could be just +2 meaning 2 holes up, or may have won "3 and 2" meaning 3 holes up with 2 holes to go (meaning the game is over as the trailing player can no longer possibly win).
I think that's all there is to it?
